MADISON and JEFFERSON COUNTY HEALTH DEPARTMENTS September PACE EH Update
The Jefferson and Madison County Health Departments have established a purchase order with the FSU College of Medicine to provide a community health assessment for each county. The list of candidates for the community based assessment team has been drafted and letters of invite will be mailed October 12. The scope of work for the COM will include the following:

- Develop a demographic profile of Jefferson and Madison Counties
- Examine the socioeconomic indicators for the counties
- Analyze the health status indicators for the two communities
- Maternal and child health
- Communicable diseases
- Morbidity and mortality trends
- Health risk behaviors
- Develop in conjunction with the community advisory group indicators for an environmental health assessment (sample indicators might include rates of septic system failures and rates of gastrointestinal illness)
- Upon completion of each of the areas bulleted above (or in whatever manner preferred), share findings with the community within each of the counties for the purpose of receiving feedback, for a total of four (4) meetings in each county
- Upon completion of the community health assessment, participate in a final community meeting to prioritize the findings
- Publish and bind the final report using a format resembling the Status of Health in DeKalb Report, 2001
- The tasks proposed are consistent with all or parts of PACE EH Tasks 2, 5, 6, 7, and 11, and will impact the related efforts for Tasks 3, 4, 8, 9, and 10
The Jefferson and Madison County Health Departments will ensure the services of an individual, at least part time, to facilitate the PACE EH process within the two counties, and develop and input local data sources as required for this project.
Primarily the data sources to be utilized for the community health assessment are assets of the Florida Department of Health and Jefferson and Madison County Health Departments.
